Nasal flu sprays will take place in school on Wednesday 2nd December for pupils in Reception to Year 6.

Consent form

You can use the link above to print and fill in the consent form if you would like your child to receive the vaccination. Please be sure to write your child`s name & class clearly.
The form must be signed by hand as the nurses will not accept a digital signature. 

You can email a scanned copy of your completed form to us at welfare@fielding.ealing.sch.uk. Please do not send a 

photo of the form. 

Paper copies of the consent form will be left with class teachers if needed.
 

All completed forms sent back by Friday 20th November.
