All parents are asked to complete this information as soon as possible and by half term at the latest. This enables all adults in school and clubs to be aware of your child's medical needs on one simple efficient system rather than relying on paper methods.

Please log on to Laser and select school forms to complete health care details. This needs to be done for all pupils even if your child has no health care needs.
